Background Questions |

When you register for the GACE, you will be asked to answer the following background questions. Providing
accurate and complete background information about yourself is important. The information that you provide
will be kept confidential.
If you register on the Internet, you will answer the following questions interactively as you register. If you register
by U.S. mail, enter your responses to the following questions on the registration form. Each question number
corresponds to the section of the registration form where you will record your response.

other than the Paraprofessional assessment. |

Indicate the educator preparation institution at which you completed or are currently completing an educator preparation
program (standard or Georgia TAPP) by entering the three-digit code from "Institution Codes."
Your test scores will be released to the Georgia institution that you indicate during registration unless
you provide to NES written notification that you do not wish your scores to be released to that institution.
Your written notification must be received by NES by one week after the test date.
Your scores will also be sent to the PSC.
If you completed or are currently completing an educator preparation program outside the state of Georgia, enter
"998" and proceed to question 8B.

Employing Agency to Receive Results |

Indicate the employing agency to receive your results by entering the three-digit code from
"Employing Agency Codes." If you enter a code, your results
will be released to the employing agency you have indicated and to the PSC.
